How can I apply for a NHFCard?
To apply for Pap Smear or PSA benefits (if you are not diagnosed with any of the chronic illnesses covered), simply complete Section A of the Beneficiary Application Form and submit to a NHFCard Centre location.
If you have been diagnosed with one or more of the chronic illnesses covered by the NHFCard programme, complete a Beneficiary Application Form as follows:
Complete Section A of the Beneficiary Application Form and insert TRN in the space provided in Section A.
Section B, should be completed by a guardian, for persons who are under 18 years old, incapacitated or under the care of an Institution.
Section C of the NHF Application form must be certified by your doctor, inclusive of the doctor’s registration number (MCJ) and signature. Your medical condition(s) must be indicated by your doctor.
Submit your application at any of the Same Day Service locations where your application will be processed within 15 minutes.
How can I apply for a JADEP Card?
The JADEP programme has been integrated under one card. Beneficiaries will be able to access the benefits eligible to them under the varying programmes (JADEP, NHF) by swiping a single card (the NHFCard).
Once a beneficiary has met the age requirement of 60, they are automatically eligible for JADEP benefits.
How can I add a new condition to the NHFCard?
Complete Section A of the Beneficiary Application Form and indicate change required by ticking the “Update Medical Record” check box.
Section B, should be completed by a guardian, for persons who are under 18 years old, incapacitated or under the care of an Institution.
Section C of the Beneficiary Application Form, must be certified by your doctor, inclusive of the doctor’s registration number (MCJ) and signature. The new condition must be indicated in this section by the doctor.
Please note that new conditions cannot be added to the database for requests received via the phone or by the submission of a prescription.
Submit your application at any NHFCard Centre locations where Change requests will be processed within 48-hours.
How do I replace my lost / stolen card?
Complete Section A of the Beneficiary Application Form and indicate change required by ticking the ‘Replace Card’ check box.

Can I use my TRN to apply for my child?
No. The parent or guardian can go to the Tax Office and apply for the child’s TRN. Ensure that you carry the child’s birth certificate.
Does a minor need a TRN to apply?
Yes. All applicants are required to have a TRN and it should be written on the application form prior to submission.
Does a senior citizen need a TRN in order to apply?
Yes, all applicants regardless of age are required to have a TRN in order to receive a NHFCard. An applicant who is unable to receive a TRN can ask about our TRN Waiver programme.
Can I get a benefit if I do not have one of the conditions covered?
Yes. You will not be eligible to apply as the NHFCard only covers benefits for the 24 chronic illnesses covered by the program. However, to apply for Pap Smear or PSA benefits, simply complete Section A of the Beneficiary Application Form and submit to a NHFCard Centre location.
Does the card expire?
No. The cards are for a lifetime. The NHFCard does not expire, the card is still valid even if an expiration date is stated. Cards are only replaced when they are lost or damaged.
I will be travelling can I get additional medication to take with me?
Approval may be granted for JADEP and NHF beneficiaries to receive more than the usual monthly quantities when travelling overseas. This amount shall not exceed 3 month’s supply. Beneficiaries requiring more than the usual month’s supply of drugs are required to submit their request in writing, stating the reason for advance. Each request will be evaluated on a case-by-case basis and approval may be granted subject to certain conditions.
For a request to be considered for approval the request should be accompanied with the following information: –
- A copy of the prescription to be filled
- Name, card number and contact number of the beneficiary
- A letter requesting the drug advance (if information is being faxed to us)
- The standard drug advance form found in NHF Apps Suite.
- A copy of beneficiary’s travel itinerary (which must have a return date).
Beneficiaries travelling on a one-way ticket overseas are not eligible for this benefit except: –
- Persons travelling overseas for medical treatment
- Persons studying abroad with valid student visa
- Persons employed to cruise lines going in position for assignments
- Persons in the Government hotel/farm work programme
Can my medication be added to my card?
No. Beneficiaries are enrolled for illnesses covered, which allows access to benefits for drugs covered on the program. If your medication is not covered, you may write to the NHF or visit the website and complete the Individual Benefits Request Form. Please see link here.
How much money is on my card?
Your card is not like a debit card, which has a dollar limit. NHF drugs are assigned fixed subsidies or discounts and the beneficiaries pay the difference between the price of the drug at the pharmacy and the NHF subsidy. Each drug has a limit to the quantity allowed during a specific period.
How often can I use my card?
The frequency with which the card can be used depends on the amount of medication received and the amount allowed. For example, if a 30-day supply of Metformin was dispensed on January 1st, the earliest date of refill would be January 26.
There are few conditions that may be exempt from this rule such as Cancer and Asthma.
If I used my NHF Card to fill my prescription for the month and have misplaced the items, is it possible to get another supply of drugs using my NHF Card?
You can make a request for a replacement refill in writing to the NHF outlining the situation along with a copy of your prescription. The written request should also contain your card details and contact information, and thereafter NHF will review and respond to you accordingly.
How long is the processing time for applications that are mailed in?
Once the properly completed application is received, it is processed and mailed to the beneficiary within 5 working days.
Can a bearer pick up my card?
Yes. For a bearer to collect your card the NHF requires a letter from you authorizing the bearer to collect. Ensure that your TRN card or application receipt is available at the time of collection.
NB. For applicants who cannot give authorization (minors, elderly or the incapacitated), the bearer must have a copy of the TRN and ID for both himself/herself and the applicant and Section B of the application form filled in.
Can my family members be added to my benefits?
No. Coverage is related to the individual’s medical conditions. If a family member requires access to NHF benefits, then, each member should apply for their own card.
Which Pharmacies accept the NHF Card?
There are participating pharmacies island wide and a full listing of all participating providers is available on the NHF website at https://www.nhf.org.jm/nhf-card/#providers-list. To identify a NHF Pharmacy, look for the NHF logo sticker at the pharmacy

If I lose my blood sugar machine or it becomes defective, before the two years have passed is it possible to get a replacement?
Yes. Kindly submit a request in writing to NHF with the relevant card details for a replacement.
Does the NHF offer Blood Pressure monitoring kits?
No, the NHF does not offer blood pressure monitoring kits.
What is the procedure for returning JADEP Expired Drugs?
In accordance with the JADEP Provider Agreement Clause 16 (2) – The Provider shall notify the Fund not less than three (3) months prior to the expiry date of any designated Item. In the event that the Provider fails to comply with this clause and as a consequence of such non-compliance the Fund sustains losses, the Provider shall reimburse the Fund for loss within one (1) month of discovery.

What is the Public Private Partnership (PPP)?
The Public Private Partnership is an agreement between the MOH, NHF and private sector pharmacies to facilitate quicker dispensing of medication to persons with prescriptions from public hospitals and clinics at specific private pharmacies. To be eligible to participate in the programme, public health patients must have prescriptions for medication that are on the Government-approved Vital, Essential and Necessary (VEN) list of drugs and be enrolled on PPP programme. The patient is expected to pay a minimal fee of $200 per prescription, which is to offset administrative costs at the participating private pharmacies.

Suppose I have diabetes but I am controlling my illness by diet and exercise may I still get a blood sugar machine through the NHF?
Yes. A letter will be required from your doctor stating that you are treating your illness by diet and exercise. Also, speak with your doctor or pharmacist about the machine that might best suit your needs. You will be contacted by the distributor of the blood sugar machines within two weeks after submitting a request to NHF, to make arrangements for collection.
What is NHF Co-ordination of Benefits?
NHF Co-ordination of Benefits enables the beneficiaries to maximize their health insurance benefits and reduce their out of pocket expenses. NHF beneficiaries with multiple cards and are filling a prescription for a drug covered by the NHF, need only present one of their cards at the pharmacy that participates in COB.
How do I become a NHF or JADEP Provider?
Kindly complete the NHF Provider Application Form and provide us with originals or certified copies of the following:
a. Pharmacy Council Certificate of Registration of the Shop
b. Pharmacy Council Certificate of Registration of the Registering Pharmacist c. Certificate of Business Registration-Registrar of Companies (if applicable)
d. Certificate/Articles of Incorporation and Articles/Memorandum of Association e. TRN (Business)
f. Police Record
g. Character Reference
It is mandatory that the owner(s)/director (s) of the pharmacy provide the NHF with a Police
Record done within the last six months (*Existing Providers on the NHF Programme who are already certified with the Pharmacy Council of Jamaica need not supply*). In addition, one character reference will be required for each of the *listed owners/directors* from the following list:
1. Attorney-at-Law
2. Minister of Religion
3. Notary Public
4. Justice of the Peace
5. Superintendent of Police or higher rank
Applications for provider status will not be considered unless these documents are up-to-date at the time of application. After review of all relevant documents submitted, if considered for acceptance, the applicant shall be offered a Provider Agreement/Contract which will specify the Provider’s contractual obligations, operational procedures and requirements of the National Health Fund. The review process takes ten (10) business days once the completed application
and all relevant documents are submitted to the National Health Fund.
How can I access a blood sugar machine?
You may obtain a glucose monitor through the NHF from one of our partners. The machines available are:

Institutional Grants
Can NHF help me with paying my tuition?
The National Health Fund (NHF) EduCare Grant Programme provides assistance to aspiring healthcare professionals who are facing financial challenges in completing their course of study. The grant is available to full-time students at accredited universities who have completed their second year of study.
What are the supporting documents needed along with the Grant Request form?
The following represents the minimum requirements for submission. Items ‘a’-‘d’ are required for all projects and items ‘e’ and ‘f’ are required for any Infrastructure and Procurement of medical equipment projects, respectively. Items ‘g’ and ‘h’ are dependent on whether additional funds are available from other institutions.
a) Project Proposal template signed by the head of the organization, e.g. CEO, President b) Completed Grant request Form
c) Project Proposal
d) Supporting documents for the cost of the project, e.g. Quotation, pro forma invoices, summary of Bill of Quantities
e) Architectural designs or working drawings relevant to the stage of the project (11” x 17” paper size)
f) Specifications for any equipment to be purchased under the project
g) Evidence of any self-financing portion of the project, e.g. Bank statement or other forms of commitment
h) Approval letter or any other supporting documents from institutions providing counterpart funding
Is there a limit on the amount given to an organization / project?
The amount to be approved will be determined by the availability of funds in each fiscal year and at the discretion of the NHF’s Board.
How will funds be disbursed?
Depending on the amount, disbursements will be made via cheque or wire transfer to the Grantee’s project account in accordance with departmental guidelines.
Can we apply for more than one project at a time?
The NHF accepts all Grant requests but approval will be based on the availability of funds in each fiscal year and the nature of the project.
What percentage will the Grantee have to contribute to the project?
The Grantee does not have to provide funds to the project, however, in the absence of monies there is the expectation of contribution in other forms, such as effort, equity, time or skill.
Can a request be made for supplementary funds to be combined with contributions from other Institutions?
A request can be made for supplementary funds, however, evidence of approval and or availability of funds from the other institutions or the Grantee must be presented e.g. Commitment Letter and/or Bank Statement.
Should the project be approved from any other institution, such as a government ministry?
It is very important that projects being undertaken in any government facility be endorsed by the head of the organization such as the Permanent Secretary or the Regional Director.
Will the project be audited?
Random audits are conducted on projects funded by the Institutional Benefits Department by NHF Internal Auditors.
Do we report on the progress of the project to the NHF?
The Grantee must provide a progress report on the status of the project to the NHF every month, no later than the 10th of each month. The reporting template is available for download via the Institution Benefits tab.
Prostate Specific Antigen (PSA) Test
What is prostate cancer?
Prostate cancer is the most commonly diagnosed cancer in Jamaican men. The low uptake of screening is a major risk factor for example only 28% of men over 40 years of age ever had a digital rectal examination done.
Prostate cancer occurs in older men, being rarely diagnosed below 40-years of age and increases in frequency as men get older.
The goal is to reduce the burden of preventable disability and avoidable premature death due to prostate cancer.
- Males 40-75 years old can now access one PSA test per year
- The subsidy on the PSA tests is set at $1,920.00 per test
- Maximum allowance is one (1) test per year
How do I access the benefit
NHFCard is required to access the benefit at provider locations.
Existing eligible individuals will have the benefit added automatically. If you do not have an NHFCard, complete Section A of the Beneficiary Application Form and submit it to an NHFCard Centre. A benefit card will be issued for use at a registered Provider.
Can I apply online?
No.
Can I get this benefit as a person who uses public sector and private sector doctors and facilities?
Yes. Enrolment and receipt of benefit is available to all men who meet the criteria.
I have private health insurance; how will this be accommodated?
Present both cards at the lab where benefits will be applied.
Does the doctor have to write a prescription / requisition form?
Yes, the doctor has to write a requisition.
What happens if I develop one of the conditions covered by NHF after enrolling for PSA?
Complete the Beneficiary Application Form Section A (and B if applicable) have your physician complete Section C and submit the form to NHF; your records will be updated accordingly and you will be able to use the same card to access the other benefit(s).
Pap Smear Benefit
Who can have the test done?
Women aged 21-65 years of age
Is there an age limit to receive the benefit for the test?
Yes.
How do I access the benefit?
NHFCard is required to access the benefit at provider locations.
i. Existing eligible individuals will have the benefit added automatically.
ii. If you do not have an NHFCard, complete Section A of the Beneficiary Application Form and submit it to an NHFCard Centre. A benefit card will be issued for use at a registered Provider.
Can I apply online?
No.
How frequently can I have the test done?
Every three (3) years.
Why is the test provided every three years as opposed to annually?
The National Screening Guidelines for Priority Non-Communicable Diseases (NCDs) in Primary Health Care in Jamaica recommends testing every three years.
I am a physician interested in becoming a provider, what’s the process?
Complete the Provider Application form- Diagnostic Services and email to [email protected]. The form is available on the NHF website here.
How much is the subsidy that NHF will cover?

Can I do the test and then claim the subsidy afterwards?
No.
